Foster Caregiver Agreement
I, (Hereinafter called "Foster Caregiver") and To The Rescue, Inc. Foster Caregiver acknowledges that he or she is not becoming the owner of any of said animals, but is willing to provide humane care for the animal. In consideration of the premises and the covenants herein contained, it is agreed between ToThe Rescue, Inc. and Foster Caregiver as Follows.
Foster Caregiver hereby accepts from To The Rescue, Inc. a certain animal and Foster Caregiver agrees to humanely take care of said animal. In no event shall Foster Caregiver become obligated or have any right to keep said animal.
Foster Caregiver agrees to not alter in any way the appearance of the animal being fostered without written permission from To The Rescue, Inc. I.e., declawing and cropping of ears or tails.
Foster Caregiver agrees to notify To The Rescue, Inc. as to any behavioral or health problems of the animal. To The Rescue, Inc. reserves the exclusive right to determine the proper course of action to take upon such notification.
The foster caregiver agrees to care for the animal voluntarily, without expectation of compensation or reimbursement, finding reward in the satisfaction and enjoyment of helping the animal of their own free will. The foster caregiver will notify the rescue of any supplies (crates, toys, food, etc) that are needed and will pick up at our storage unit located in Springfield, VA.
If Foster Caregiver wishes to adopt said animal, Foster Caregiver must go through To The Rescue, Inc. Adoption Program Screening Process. To The Rescue, Inc. reserves the right to determine final disposition of the animal.
If the foster animal is lost for any reason. The foster caretaker will be responsible for paying To The Rescue, Inc. in the amount of the adoption fee (Cats/Kittens) $150-$300, Dogs/Puppies $350-$500)
Foster Caregiver must give To The Rescue, Inc. one week notice if unable to care for the nimal further to allow time to find a new placement.
To temporarily house and care for any dog/cat belonging To The Rescue, Inc. Caretaker understands that To The Rescue, Inc. is the sole and rightful owner of said foster dog/cat.
I understand a To The Rescue, Inc. representative may visit my home for a home inspection before my foster application is approved and at any time during fostering.
I understand that it is my responsibility to report any unusual medical or behavioral issues that may be of concern as soon as possible.
I understand that it is my responsibility to provide regular updates and photos of my foster(s) whether or not I am contacted by an authorized personnel of To The Rescue, Inc. to make sure the organization is kept informed.
If for any reason caretaker is unable to continue to house and care for said dog/car, that he/she will notify To The Rescue, Inc. in a timely manner and will return said dog to To The Rescue, Inc. Caretaker understands that said dog might then be placed in a new foster home, in a boarding facility, or be euthanized if no other solution is found.
To The Rescue, Inc. will pay all expenses or reimburse caretaker "only" if said expenses and vet clinic have been approved by To The Rescue, Inc. (unless caretaker prefers to be financially responsible for said dog). Caretaker keeps all receipts for reimbursement.
Caretaker must give the rescue a minimum of a two-week notice if they can't foster any longer to make arrangements for another foster home. If there is a date you must cease fostering, this must be made known at the start of fostering.
Foster Caregiver agrees to let To The Rescue, Inc. inspect foster caregivers premises where the animal is being kept at any time to ascertain and satisfy itself that the animal is well cared for.
Foster Caregiver agrees to provide adequate food, water, shelter and kind treatments for said animal at all times. In addition, Foster Caregiver must adhere to all state and local animal laws. Foster Caregiver agrees to follow all additional written instructions from To The Rescue, Inc.
Foster Caregiver will notify To The Rescue, Inc. in the event any change occurs in the address or telephone number. Foster Caregiver understands and acknowledges that he/she does not have any right or authority to keep the foster animal or place foster animals in other homes or place with other individuals unless permission is given in writing by to The Rescue, Inc.
The foster caregiver acknowledges the risk of animal bites and other injuries, agrees to not hold To The Rescue, Inc. liable for such incidents, and will indemnify the organization against any related claims. Additionally, To The Rescue, Inc. is not responsible for any property damage, diseases, or parasites transmitted to other animals owned by the foster caregiver.
I will return all supplies provided by To The Rescue, Inc. in a timely manner.
To The Rescue, Inc. will be responsible for vet care for the foster animal with arranged vetting by To The Rescue, Inc.'s vets only. If you choose to take your foster animal to your vet not approved by To The Rescue, Inc. you will be responsible for the bill.
I will at no time assert any claim, charge, or demand of any kind or nature. I agree to pay To The Rescue, Inc. the sum of $750 as liquidated damages in the event the terms of this contract are breached including return of the animal. I agree to pay court costs in the event this matter is forwarded to an attorney for law enforcement.
To The Rescue, Inc. does not warrant the temperament or behavior of said foster dog/cat and To The Rescue, Inc. is not to be held liable for any acts of said foster dog/cat while in the custody.
To The Rescue, Inc. is not responsible for any damage caused by said foster dog/cat to caretaker's property or possessions.
All animals belonging to caretaker personally which are not the property of To The Rescue, Inc. shall be spayed or neutered. Exceptions could be made for senior pets.
In the event a foster dog/cat becomes lost or is stolen, caretaker shall immediately notify To The Rescue, Inc.
Foster dogs/cats are to be supervised at all times, whether indoors or outdoors. Foster dogs/cats must be supervised at all times when children and other animals are present.
Foster dogs/cats are never to be left outdoors while caretaker is away from home unless kept in a secured kennel approved by To The Rescue, Inc. and only in good weather.
Caretaker shall make said foster dog/cat available to any prospective adopter at a mutually convenient time and location.
Caretaker may permanently adopt said foster dog/car if such placement is approved by To The Rescue, Inc. and provided that no pending applications are in process for the same dog/cat.
SIGNATURE: I AGREE THAT ALL STATEMENTS IN THIS APPLICATION ARE MADE BASED ON PERSONAL KNOWLEDGE AND ARE MADE FOR PURPOSES OF MY APPLICATIONS TO FOSTER ONE OR MORE ANIMALS THROUGH TO THE RESCUE, INC. FOSTER CARE PROGRAM.
